Your Amputee e-card will be emailed to you within 5 working days of payment and required information


  • * Your Passport Style Photo ( Jpeg / PNG ) for your e-card should be sent separately 
  •    with your First name and Surname to   amp.ecard@protonmail.com
  •   ( including your photo and/or your passport number on your e-card is optional )

2:  First name and Surname.

3: Amputation type and level ( below knee / above knee )

4: Amputation - Right or Left leg.

5: (Optional ) 'Notification of amputation status' text in which additional Language, for travel abroad or your National language.

6: ( Optional ) Passport Number and Nationality .
